Transfer of Medical Records:

If you are a current patient of Dr. Yamini Maddala, your medical records will remain confidential and secure in the custody of GI Alliance. If you would like a copy of your records or wish to have them transferred to another physician or practice, you may submit a request online directly to HealthMark at https://healthmark-group.com/patients/ or by contacting the HealthMark medical records department at 1-800-659-4035.
