- The front desk at any of our offices will have a medical records release form ready for you to sign. This is the fastest and most efficient way.
- The easiest way is to go to the website www.yaminimaddalamd.com, and click on the “Transfer Medical Records” button on top right corner of the home page.
- Not really! It takes 2 minutes to fill it. You have to fill out only the Patient Information section (below) of the Medical Records Release form .

Please request that your medical records be transferred to Dr. Maddala.
Dr. Maddala would require your medical records before she sees you as a patient, before your procedures, or before requesting prescriptions.
The process to obtain your Medical Records from GIA/another organization or provider can take between 1 week and 2 weeks.
Hence, please request your medical records from our office as soon as possible to avoid any disruptions in your care or prescriptions.
Our office staff faxes your Medical Records Transfer form immediately to HealthMark for processing.
Healthmark streamlines the release of medical records by sending this request to the custodian of medical records, i.e., GIA, any organization or healthcare provider of your health records.
Healthmark notifies us once they receive your medical records, and our staff will securely download the medical records, and attach them to your chart.

Please ask the front desk personnel for a “Medical Records Transfer Request” form at one of our offices. The staff will provide you with a pre-filled form that you can sign.
